ABOUT

The Healthier You: NHS Diabetes Prevention Programme is a behaviour change programme.

This means that it focuses on looking at the factors that are increasing your risk of Type 2 diabetes and helping you to develop a plan to make healthy lifestyle changes that will reduce or remove your risk.

What can you expect on the programme?

You will be invited to join a small group of people who have also been identified at being at risk of Type 2 diabetes. The group will be held at a local venue and led by a trained facilitator. The sessions are informal, with the facilitators being friendly, local individuals. Sessions will include learning about nutrition and cooking making healthy food choices and increasing your levels of physical activity. Following these meetings, you will receive further support with your progress on a regular basis, you will also be given information about a range of relevant local activities.

What will the programme cover?

  • Information on diet

  • Information on physical activity

  • Managing stress and your emotional wellbeing

  • The importance of monitoring your weight

How can you join the programme?

You can access the Healthier You: NHS Diabetes Prevention Programme in the following ways:

 

  • Your GP or nurse may have told you that you are at a high risk of developing Type 2 diabetes - ask them to refer you to your local Healthier You service

 

  • You may have discovered in your NHS Health Check that you are at a high risk of developing Type 2 diabetes - ask the Health Check professional to refer you to your local Healthier You service

 

  • You may have had your risk assessed at local events or by using an online assessment tool - if you think you are at a high risk you can ask your practice nurse to arrange the necessary blood tests and if the risk is confirmed you can ask for a referral to your local Healthier You service
