Appointments

We want to deliver services at a time and place that is convenient to you.

 

Health Exchange CIC accepts that circumstance can change and service users need to change their appointments some of the time Health Exchange CIC would therefore expect its service users to undertake the following actions if unable to attend an appointment, this will enable the delivery of a quality service to all service users.

  • If you are going to be late- please telephone 0121 663 0007 (press option applicable to service) or Free-phone 0800 158 35 35 or call the staff member you have the appointment with on the number you have been given. If you do not have their direct contact details, please use the details above.

 

  • If you are going to be a few minutes late we will try to keep your appointment available

  • If you are not able to attend your appointment -Please try to let us know at least 24hours prior to the appointment by phoning the following – 0121 663 0007(press option applicable to the service) or Free-phone 0800 158 35 35 or call the staff member you have the appointment with on the number you have been given. If you do not have their direct contact details, please use the details above.

 

  • If it is your first cancelled appointment then we will try and give you another appointment as soon as possible

  • If you miss two or more appointments – We hope that you will make every effort to attend a second appointment. Please however be aware that if you fail to attend a second appointment then we will assume that you no longer require our service and a further appointment may not be made with you

 

  • If you miss three appointments- or if you consistently fail to attend appointments then we may have to exclude you from our services. We will always notify you in writing if we intend to do this and the service user is always given an opportunity to respond in writing to us.
