JOB TITLE: Compliance and GP Liaison Officer
SALARY: £26,000 to £28,000, plus car allowance
REPORTS TO: Support & Performance Manager – North Region
LOCATION: Home Working/Head Office
HOURS: 37.5 Hours per week

LWTC is a charity set up by Health Exchange and Westbank Care Services to provide pre-type 2 diabetes services. The vision of the partners is to grow both vertically and horizontally increasing our area of business throughout UK. LWTC is investing in and operating regionally to deliver the National Diabetes Prevention Program.

Job Summary

To oversee the Locality Coaches to ensure effective delivery of the NDPP service in the Central Region, encompassing Derbyshire and Nottinghamshire

To ensure Locality Coaches meet their activity targets and key performance indicators (KPIs) as detailed in the service specification.

To engage with Clinical Commissioning Groups, Primary Care Networks, General Practices, and other key stakeholders locally to facilitate the contracted level of referrals are made to individual Locality Coaches.

To liaise and network with the local Health Community to publicise and oversee the Central Region.
Principal Responsibilities

Deliver training to Locality Coaches, to ensure they are competent to deliver the service in line with the agreed program.

Maintaining training records and ensure Locality Coaches maintain their mandatory and operational training requirements.

Monitor performance by undertaking regular audits to ensure Locality Coaches adhere to service delivery standards.

Undertake monthly performance meetings to review performance against individual KPI targets.

Provide reports on the performance of individual Locality Coaches.

Work with the Support & Performance Manager to address issues of underperformance of individual Locality Coaches.

Support and engage with PCN’s, General Practice and primary care networks to ensure the agreed level of patient referrals.

Manage and monitor the appropriate level of incoming referrals to each individual Locality Coach to ensure they meet their overall retention targets.

Monitor the Central region to ensure the overall referral and retention activity targets are achieved and ensure monthly referral targets are met.

To ensure the appropriate level of referrals are made to our Digital Partner, Liva Healthcare.
