Living Well Taking Control has a new and exciting employment opportunity to announce. We're looking for a full-time Prevention Facilitator capable of:

​

• Supporting General Practices across Derbyshire to refer eligible patients onto the Healthier You NHS Diabetes Prevention Programme.

• Raising awareness of the Healthier You Programme amongst primary care staff.

• Improving the quality of referrals by uncovering existing barriers to referral and working with General Practice to overcome these challenges.

​

The successful candidate will be an invaluable member of the Derbyshire Living Well Taking Control team, helping local communities to reduce their risk of developing diabetes. A full induction and training plan will be provided to ensure the successful candidate has everything they need to flourish.

​

Experience of working within, or alongside, General Practices in Derbyshire is desirable. Car ownership and full clean driving licence is essential.
