Recruitment of General Practice Support Role

Living Well Taking Control is seeking to recruit a Prevention Facilitator to support General Practices across Derbyshire in referring eligible patients onto the Healthier You NHS Diabetes Prevention Programme. We’re seeking candidates who have experience of working within Derbyshire primary care. Details on how to apply can be found here.

Self-Referral Pathway Extension

At-risk adults across Derbyshire now have until 30th September 2021 to self-refer themselves onto the Healthier You Programme. A “Know Your Risk” tool is accessible here and eligible adults scoring 16 or over will be able to register for free support.

This referral route is available to people who have not had a recent blood test and General Practices can promote this route to various patient populations.

Diabetes Prevention Week

We’re working to hold an event for primary care stakeholders to coincide with Diabetes Prevention Week in May 2021. The event will feature early outcomes, support in referring eligible patients, and feedback from patients on the programme. Invites will be on their way soon!
