Your local Healthier You programme is provided by Living Well Taking Control. The aim of the NHS Diabetes Prevention Programme is to help local patients improve their knowledge, ability and confidence whilst offering the support needed to make positive lifestyle choices.
For GPs
How to refer
You can refer patients to the NHS DPP directly through the general practice clinical system. Send patient referrals to our secure nhs.net email account.
We also support general practices in mailshotting patients to self-refer into the service.
Referral criteria
To be eligible for the programme, patients must meet the following criteria:

Aged 18 and over

Not pregnant on referral

Have a valid blood measurement taken within 12 months of referral

Patients must have blood measurement including:
1. HbA1c of 42 – 47 mmol/mol (6.0 – 6.4%)
2. FPG of 5.5 – 6.9 mmol/l3.
3. Oral Glucose Tolerance Testing (OGTT) two hour post 75 gram glucose load glucose value ≥7.8 and <11.1 mmol/l

How to enrol
If you think you are at risk of Type 2 diabetes, or are showing the symptoms, speak to your doctor so they can arrange an assessment.
If you qualify, you will enrol on the NHS Diabetes Prevention Programme.
This support programme gives you everything you need to lead a healthier life - helping you to eat better and move more.
Session are run by local health coaches in your local area.
You will also have access to an app to keep track of your health goals.

Where the sessions take place
To protect participants and coaches during the Coronavirus pandemic, we are changing the way we deliver the Healthier You: NHS Diabetes Prevention Programme. Participants will be invited to access remote sessions delivered by their coach via the internet, or via landline telephone from the comfort of their own home.
Once Coronavirus social distancing measures have been lifted, face-to-face group sessions will be delivered in the local community as normal. In future, this section will include an interactive map showing where we deliver group sessions in your local community. We often deliver group sessions in venues such as libraries, community halls, places of worship and general practices.
