Living Well Taking Control has been commissioned to deliver the Healthier You: NHS Diabetes Prevention Programme across Southport and Formby.
If you are over 18 years old and have had a blood test within the last 12 months which indicates you are at a high risk of developing Type 2 diabetes (the most recent blood test will be used) then your GP may refer you to the programme.
GP practices are currently working to identify eligible patients.

What can I do myself?

Take a Risk Assessment

Find out whether you are within the recommended weight range for your height

If you are overweight, adjust your diet to reduce weight

Change your diet permanently to ensure you are following the guidelines for healthy eating and alcohol consumption

Ensure you are taking the recommended amount of exercise - 'sedentary is the new smoking!' research shows that being sedentary poses a significant risk to health
