OUR PROGRAMME

We offer two options for taking part in the Healthier You: Type 2 Diabetes Prevention Programme.

1. You can join our coach-led group programme alongside others in your local area who are also reducing their risk of developing Type 2 Diabetes.

2. We offer an online app alternative, so you can complete the programme remotely with digital support from your local health coach.

This is a free, 9-month programme designed to significantly reduce your risk of developing Type 2 Diabetes and more generally improve your health.

LUKE FRAMPTON
Hereford & West Essex GP Liaison Officer

Luke has worked within the NDPP for over 5 years, from a health coach to his current role as a compliance and Liaison officer. Luke loves how rewarding it is be to working in a team that helps others to achieve their health goals to the best of their abilities, and seeing the positive outcomes we can have on peoples lives.

Coach

Penlin is a qualified Nutritionist with a BSc in Human Nutrition science. She has been a Health Coach for the 5 last years, helping people to create healthy habits that are manageable. Penlin enjoys supporting people to overcome obstacles to behaviour change and manage weight & stress - to improve blood glucose control. 

Your local Healthier You programme is provided by Living Well Taking Control. The aim of the NHS Diabetes Prevention Programme is to help local patients improve their knowledge, ability and confidence whilst offering the support needed to make positive lifestyle choices.
