Join the Healthier You NHS Diabetes Prevention Programme 

Eligibility Criteria

The programme is open to people who:

​

  • live in one of the areas listed below

  • aged between 18 and 79 years old

  • are not pregnant

  • do not have diabetes currently

  • and have received a score of over 16 when using the Know Your Risk tool

Your risk score indicates that you are at high risk of developing Type 2 Diabetes.

 

Please contact your General Practice to express your interest in joining the Healthier You Diabetes Prevention Programme.
